Isometamidium acts as a cationic compound, exhibiting high affinity for the DNA of the Trypanosoma parasites. It intercalates (inserts itself) into the parasite's kinetoplast DNA, specifically preventing DNA replication and transcription, which is essential for the parasite's survival and division. This results in the rapid death of the parasite. Crucially, the compound is slowly released from the injection site and strongly bound to tissues, providing a prolonged concentration in the bloodstream, which is responsible for its effective prophylactic (preventative) action.
